Failure to closely follow specific rules, regulations, and confined space safety standards can just about guarantee serious injury or even death. The National Institute for Occupational Safety and Health or NOISH found in more than 50 percent of confined space accidents, the rescuer becomes the victim. NIOSH also found that very few organizations provided any type of written procedures or training specific to confined space entry.
